Annex IV. Inspection Checklist: Used Machinery and Equipment

               5. Dangerous/Moving Parts of Machinery or Equipment
                                                                                     Y N N/A

Are guards and protective devices installed on moving parts of
                 machinery or equipment?

                    - moving transmission parts
                    - moving parts involved in the process

Are the guards and protective devices robust and their functions well maintained?

                    - Fixed guards

                    - Interlocking guards

                    - Adjustable guards

Do guards and protective devices cause minimum obstruction to the view of the
                 production process?

Is it not easy to bypass or render non-operational?

Is there measure to prevent accidental generation of unexpected start commands
likely to cause dangerous machine behaviour? (Note. Start-up by the normal
operation of automatic machinery is not to be considered as unintended start-up)

Are there maintained stop commands (for example, interlock) to prevent restarting
that could result in dangerous machine behavior?

                    - Is there a readily accessible stop device (normal stop, operational stop,
emergency stop) which stops the machinery or equipment in a safe way?

Is emergency stop control placed within immediate reach of the operator?

                    - Does the machinery’s or equipment’s stop control have priority
                      over the start controls?

                    - Does the control or operating mode selected override all other control or
operating modes, with the exception of the emergency stop.

                    - In case of machinery or equipment designed to work together, Can the stop
controls stop not only the machinery or equipment itself but also

                      all related machinery or equipment?

                    - Can controls be operated safely and easily, located outside the danger zones?

                    - Is there any system of work which ensures that nobody is in a dangerous
position when machinery or equipment is about to be started?

                                                                                                        107
