Elevated miR-93-5p from extracted thrombus is associated
with recurrent vascular event after cerebral infarction

Background: We investigated whether microRNA expression pattern can provide information about neurological
deterioration among acute ischemic stroke patients. Methods: Between 2015 and 2020, acute ischemic stroke
patients who had been through mechanical thrombectomy from Chung-Ang University Hospital were included.
After informed consent we harvested thrombus at -70℃. The expression signature of microRNAs was evaluated by
microarray analysis. The significantly altered microRNAs were validated by quantitative real time polymerase chain
reaction. Correlation analysis was performed to study relationship between significantly altered microRNAs and
laboratory variables. Results: Among 95 patients who received mechanical thrombectomy, a total of 78 patients
were finally included in the study after excluding 9 patients without thrombus and 8 patients without undetectable
microRNAs. Recurrent vascular event was detected in 13 patients. Microarray analysis comparing 1088 microRNAs
between the patients with and without recurrent vascular event and validation by RT-PCR showed that miR-93-5p
and miR-20a-5p were significantly elevated among the patients with RVE. Both microRNAs were significantly
elevated among the patients with poor prognosis and positively correlated with discharge NIHSS. Correlation
analysis found that the expression levels of miR-93-5p was positively associated with E/e’ ratio and serum
parathyroid hormone. The expression of miR-20-5p was positively associated with platelet count,
